Community Health Worker Program
About the Program
CHWTP provides the necessary MiCHWA-approved training to become a Community Health Worker (CHW), Health Support Worker, or Community Health Aide. Through the specialized education, students concurrently complete 126 hours classroom-based training and 40 internship hours. Students will use their training to become frontline public health workers who are trusted members of the community or who have a close understanding of the community served.
Financial support is available to qualifying candidates who successfully complete the program.
Program Requirements
- At least 18 years of age
- High School diploma or GED (minimum)
- CHWTP Application form
- Basic technology skills (email, navigating the web, working with files, etc.)
CHWTP Apprenticeship Program
Apprenticeship Description:
Applicants for the ‘Editable Section’ CPRM Academy cohort must submit their application (and all accompanying documents) by the ‘Editable Section’. Those with incomplete applications will not be accepted. The required application materials are:
- CPRM Academy Application Form
- Personal Interest Statement
- Proof of Education
- Reference for Involvement in Recovery Community
Program Prerequisites:
- Completed CHWTP in good standing
